WAIVER AND RELEASE OF LIABILITY FORM

Please read this waiver and release of liability form carefully. By participating in any activities or using the facilities at The Dojo MMA Inc., you acknowledge and agree to the following:

1. Assumption of Risk: I understand that participating in martial arts training and related activities involves inherent risks, including but not limited to, physical exertion, contact with others, and the use of equipment. I voluntarily assume all risks associated with my participation and acknowledge that these risks may result in injury, illness, or property damage.

2. Release of Liability: In consideration of being allowed to participate in martial arts activities at the gym, I hereby release and discharge the gym, its owners, instructors, employees, and affiliates from any and all liability, claims, demands, actions, or causes of action, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my property while participating in martial arts activities or using the gym facilities.

3. Waiver and Indemnification: I agree to waive and release any and all claims against the gym and its representatives, and I agree to indemnify and hold them harmless from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, arising out of or related to my participation in martial arts activities or the use of the gym facilities.

4. Medical Considerations: I affirm that I am in good physical condition and have no known medical conditions that would prevent my safe participation in martial arts training. If I have any medical concerns or conditions, I have consulted with a healthcare professional and have obtained their approval to participate.

5. Photography and Media Release: I grant the gym the right to take photographs or videos of me during training sessions or gym events. I authorize the gym to use such photographs or videos for promotional or educational purposes, without compensation or notification.

6. Severability: If any provision of this waiver and release of liability form is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I have carefully read and understood this waiver and release of liability form. I voluntarily agree to its terms and conditions and acknowledge that I have signed this document freely and without any inducement or assurance of any nature.
